Activity Report Instructions

Section Name/Br No. Select from drop-down list.

Type of event.

SAR - Use for all searches. You will need to obtain extra information from your search controller to fill out this form. You will need the NZMG (NZMT from September 2009 ) Grid Ref, Police Event number or RCCNZ Tasking number, Number of people At Risk, Saved, Rescued, Assisted, Perished or Unaccounted for, details of VHF/HF equipment used and details of other services provided.

SAR MEETINGS & FORUMS - Use for all SAR related meetings or forums Use this event type for all occasions where expert advise has been given.

SAR TRAINING - Use for all SAR related training. Also use for SAREX operations.

CD - Use for all CD operations

CD TRAINING - Use for all CD Training and exercises.

RURAL FIRE - Use for all Fire callouts.

RURAL FIRE TRAINING - Use for all Fire training.

SPORTS & TRAINING - Use for all sporting activities and section training.

ADMINISTRATION - Use for all AREC administration, AREC Nets, Meetings, Planning etc. There is a lot of unreported time put into AREC which is not directly assignable to the other event types.

Advised by/Time advised - Enter if known.

Date of Activation - Enter date of activation. For multiple day events, enter a separate Activity Report for each day.

General location of Event or SAROP - For SAROP, The location of where a person or people were rescued from OR, if no rescue occurred, the last known location of the missing person(s).

Grid Reference.- Enter the Map Sheet number eg BJ29 and NZMG/NZMT grid reference eg. Wairongionai Track, Rimutaka Range 2688290 5992594.

Nature of Event - A short description of the event or SAROP.

Result of Activity - A brief report on the activity. Longer reports should be sent to the AREC Break-In Scribe.

Callsigns of members involved. - Enter the callsigns of all involved. Enter names for those without callsigns. Separate each callsign with a comma and a space eg. ZL1ABC, ZL1DEF, ZL1GHI.

Number of Operators.- Enter the number of Operators involved.

STATISTICS

For SAR/ SAROP's the following information is required for each event.

Category of SAROP. - Select from drop down list and enter the appropriate Police Event number eg. P002495428 or RCCNZ Tasking number eg. 1234/09

Number of people designated as:

AT RISK - A number that totals the following sub groups.

SAVED. - Lives saved: Where if SAR agencies had not intervened, life would definitely have been lost.

RESCUED.- Lives rescued: Where SAR agencies locate and rescue a person or people at risk and return them to a safe location.

ASSISTED.- Lives assisted: Where SAR agencies aid a person or people at low risk but who, if left, would be at risk.

PERISHED. - Number perished: Where SAR agencies respond to a incident and locate a person or people who have perished.

VHF/HF Resources used. - Significant resources utilised for the SAROP.

Any other services provided by AREC for the SAROP. -Record any Expert advise given, field team communications training, issuing and testing radios prior to team deployment, assistance for siteing repeaters, runners, catering etc.

Duration of event/Total man hours. - Enter the total number of volunteers used during the event/SAROP and the total number of hours volunteers spent conducting the event/SAROP.

Value of Equipment Used. - Enter a estimate of the value of equipment supplied by AREC, Police & SAR, CD or Private & other used directly for the event/SAROP.

Prepared By.- Enter your Name, Callsign, Date and email address. Please double check that email address and Name details are correctly spelt. For SAR events, these details are sent as part of the SAROP report.

A copy of the report will be sent to the email address entered in the Activity Report. For SAR events a second SAROP report will also be sent. Please forward this to your Search Coordinator.
